What is an Annual Wellness Visit?
The Annual Wellness Visit is a special benefit offered to Medicare patients at no additional cost. This visit is designed to focus on preventive care and help you stay healthy. During this visit, you will have the opportunity to discuss your medical history, current health status, and any risk factors for certain diseases with your healthcare team. Together, you will create a personalized plan to maintain and improve your health.

How is it Different?
The Annual Wellness Visit focuses on preventive care and health planning rather than performing a full physical exam. During this visit, we will discuss your medical history, current health status, and risk factors, but we will not conduct routine physical exams, such as listening to your heart and lungs, or perform routine screenings and blood tests. The goal of this visit is to proactively manage your health by creating a personalized wellness plan.
During your visit, we will:
Discuss your medical history and any concerns you may have.
Evaluate important health indicators such as height, weight, and blood pressure.
Offer referrals for necessary screenings or additional services tailored to your needs.

Common CPT Codes for Medicare Annual Wellness Visits include:
- G0402 – Initial preventive physical exam (IPPE), also known as the "Welcome to Medicare" visit

A Annual Wellness Visit is a comprehensive appointment designed to assess your health, discuss your medical history, evaluate your risk for certain diseases, and plan strategies to maintain your well-being. It focuses on preventive care rather than conducting a full physical examination.
Unlike a routine physical exam, the Annual Wellness Visit is centered around discussing your health history and preventive care. We won’t perform a full physical examination or order routine screenings during this visit. Instead, we will measure vital health indicators like height, weight, and blood pressure, and discuss any health concerns you may have.
You are eligible for a Annual Wellness Visit if you are enrolled in Medicare Part B. You can have your first visit within the first 12 months of enrollment and then annually thereafter.
To make the most of your visit, please bring:
- Completed forms provided in advance.
- A list of your healthcare team members, including specialists.
- Information about your home health agency and medical equipment suppliers.
- Details of your pharmacies.
- A bag containing all medications you take, including over-the-counter drugs, vitamins, and supplements.
Medicare covers the cost of the Annual Wellness Visit, so there is no out-of-pocket expense for you. However, if additional tests or services are needed during the visit, you may have a copayment, and the Part B deductible may apply.
If we determine that you need additional screenings or tests, we may refer you to appropriate services outside of the Annual Wellness Visit. Some of these services may require a copayment.
While the focus of the Annual Wellness Visit is on preventive care and health planning, you are encouraged to discuss any new health concerns. Depending on the nature of your concerns, we may schedule a separate appointment to address them thoroughly.
